A Health Risk Assessment (HRA) is a short, confidential questionnaire that looks at your lifestyle habits, personal health history, and key risk factors. It's designed to give you a snapshot of your overall health and areas where you may benefit from extra support.
The results can then guide you toward programs, coaching, or chronic disease support that are tailored to your needs. By identifying risks early and making small, meaningful changes, you can prevent bigger issues in the future and improve both your long-term health and quality of life.
The results of the HRA are not shared with your employer but may be used to determine if you are eligible to participate in health management coaching.
